Keep track of assignments that you complete each week, using this record sheet. Bring a copy to your midquarter conference and final eval conference. Be honest about the quality and completeness of your work and participation; your records should match the subset which your faculty records.

TM = team meeting, SPIQ = Seminar Points, Insights, and Questions (WebX), EPIQ = Energy Points, Insights, and Questions (WebX), PA = Participation and Attendance, E=weekly Essay (WebX), R = weekly research post (WebX).

Mark (+) for fully complete work of high quality; check (/) for substantially complete work of acceptable quality, (-) for incomplete or poor work, (0) for missed assignments or attendance, (L) for late assignments or late attendance, (ex) for excused absence. If you have numerical scores, say for homework or exams, do include those (e.g. 82% or 14/17). Note any special circumstances that your faculty should know about, such as excused absences (e.g. surgery, week 4). You can search for your name (or your team) in WebX to get summaries of your posts.

For classes, mark (+ / - 0 L ex) as appropriate for your attendance and participation. For debates, mark P when you argue the Pro side (and indicate your topic), C when you argue Con, F if you facilitate. For seminars, mark F when you facilitate (F is good.) Leave the cell blank (or shade it) when we do not have a given assignment on a given week.

NA = Not Applicable (e.g. no such assignment that week - don't worry about it)
