Keep track of assignments that you complete each week, using this record sheet. Bring a copy to your midquarter conference and final eval conference. Be honest about the quality and completeness of your work and participation; your records should match the subset which your faculty records.

TM = team meeting, PIQ = Points, Insights, and Questions (WebX), PA = Participation and Attendance, Essay=Essay (WebX), Res = response to peer essays (WebX).

Mark (+) for fully complete work of high quality; check (/) for substantially complete work of acceptable quality, (-) for incomplete or poor work, (0) for missed assignments or attendance, (L) for late assignments or late attendance, (ex) for excused absence. Note any special circumstances that your faculty should know about, such as excused absences (e.g. surgery, week 4). You can search for your name (or your team) in WebX to get summaries of your posts.

In seminar, mark F when you facilitate (F is good.) Leave a cell blank (or shade it) when we do not have a given assignment on a given week.

NA = Not Applicable (e.g. no such assignment that week - don't worry about it)

Whether you are in Monday seminar, Thursday seminar, or both, please turn in ONE portfolio in class week 9; you will have one team conference in week 5, and one individual conference in week 10.
