The Theory and Practice Form is only in PDF format and there isn't any easy way to convert it.

I apologize to anyone who needs it in .doc or .rtf.

I suggest that you download it on campus and print it from one of their free printers.

But don't print it from one of their pay per print printers. Those things are just too expensive.
