How to Create Your Artist Packet
Below is a download, "How to Create an Artist Packet," from the book I'll accurately title later.
Also, a great (!!!) website:

Go under Guidelines, and under Career Development, there are wonderful examples for you to look at.
As for artist statements, check out the statements of the artists you admire, check out the ones under resources. They can be anything and everything, describing your entire oeuvre of work, or one specific piece. It can be totally descriptive of technique, poetic musings, or abstract agendas. Whatever you feel is a best, or most applicable "statement" of your art or yourself as an artist.
Good luck! Write away! We can help each other during a workshop period in the future.
